Victim Services - Enroll for Notification

Registration is not automatic - participation in the program is voluntary.

How To Apply

  1. Fill out the Victim Services Application. Even if you do not have enough information to fill out the entire application (you must at least have the offender's name), you may still submit an application.
  2. After eligibility is determined, you will be notified of your enrollment in the Victim Services Program by mail. If at any time you wish to discontinue your participation in the program, you may do so by requesting to be removed from the Victim Services Program.

Let Us Know When You Move

If your address or phone number changes, call us at (800) 322-2201 or email us to let us know. The Victim Services Program will not be able to notify you of the offender's status if you do not report changes in your address or telephone number.

All information you submit to us is confidential, and will be safeguarded. Your information will not be available to the offender.
